Once you’ve made an appointment, kindly complete the Patient Registration Form and Fertility/Health Questionnaire which have been sent to you or a downloadable copy can be found below. Please return the completed documents to [email protected] prior to your consultation.

With information obtained beforehand, we aim to maximise your time with Dr Fong by targeting issues important to you. 

What to expect on your first visit

​The consultation time is generous and adaptable to the nature of your visit, which could be for:

• General fertility work up and management
• Second opinion
• Prompt and streamline access to start IVF or ICSI (if appropriate)
• Exploring specialty services such as pre-implantation genetic testing or donor egg/sperm program
• Endocrinology

What to bring

A) Registration and payment

• Referral letter addressed to Dr Fong from your GP or Specialist with you and your partner's full name  
• Medicare cards and a photo ID for both of you
• We accept cash, EFTPOS and credit cards (except AMEX) 

 
B) Fillable Forms

• New patient registration (to be completed by each partner)
• Fertility & health questionnaire (to be completed jointly)

C) Medical information & investigations

• Previous test results including blood tests, pelvic ultrasounds, sperm and other relevant health information
• Previous fertility treatment reports such as cycle protocol, medications, dosages, follicle tracking, hormones, laboratory findings, embryo grading and treatment outcomes
• Correspondences from treating specialists (eg geneticist, gynaecologist, urologist, endocrinologist)
• Surgical findings and histopathology reports
